At Heartland Wellness Connections (HWC), we believe better health starts by meeting people where they are.
We are a community-rooted nonprofit focused on improving health access, digital equity, and wellness across Cass County and surrounding communities. Our work connects people to the tools, education, and support they need to live healthier lives—whether that means managing diabetes, learning technology skills, accessing care, or finding trusted community resources.
From Tech Tutors and Tech Café to diabetes education, maternal health support, and community health worker outreach, we build practical solutions that remove barriers and improve lives.
We are not a clinic, we are a connector, a convener, and a catalyst for healthier futures.
